School Year 2008-2009 Immunization Requirements

All children need to have an up-to-date copy of immunization records at their school site to be enrolled in and attend school. If your child's immunizations need to be updated and you bring in written proof of an appointment within 20 days of the start of school, you may temporarily enroll your child pending receipt of the required immunizations. Your doctor or the Baltimore City Health Department can provide the DHMH 896 Form or a computer-generated form for you to take to the school.

Recent immunization requirements for Hepatitis-B and Varicella (chicken pox) affect students in grades Pre-K through 11th grade for school year 2008-2009, and the requirements are new for 11th graders this year.

For school year 2008-2009, students who need shots will be temporarily admitted through Friday, September 12, 2008. Per Maryland State law, students who have not received required shots will not be permitted to attend school starting on Monday, September 15, 2008. So Don’t Wait to Vaccinate.

At some schools in Baltimore City, students will be able to receive FREE Hepatitis-B and Varicella vaccines from the school nurse, with a signed parental permission form (download a copy below).  Contact your child’s school to see if they offer this service.  The Baltimore City Health Department also offers FREE vaccines to students at their community health clinics.

  • Your child’s school
  • The Baltimore City Public School System’s Command Center at 443-984-2000.
  • The Baltimore City Health Department at 410-396-4454 for FREE shots.
