Immunization Requirements

Students in a computer lab working Maryland law requires that students be fully immunized against  certain communicable diseases in order to attend school.  Students and parents must make sure that each student's  school record on immunization contains accurate and  up- to- date information and satisfies the State Health  Department requirements.

The Maryland State Immunization Regulations require that  children who are starting school have the appropriate  immunization shots. For each new entrant, a Maryland Immunization Certificate, Form 896, is required by law so that immunization information may be recorded in the school file. For more information, please call 410-396-8995.
