* Request For Course Approval For Tuition Reimbursement (To gain approval for tuition reimbursement). Transcript, Grades, and Tuition Payments must be attached with the form. This forms should be submitted to the Office of Certification.

* Tuition Reimbursement Policy and Procedures (Explains the Tuition Reimbursement process)
This forms should be submitted to the Office of Certification.

* Tuition Reimbursement Form for AFSCME Employees

* Tuition Reimbursement Form for BTU Paraprofessional Employees

* Tuition Reimbursement Form for BTU Teacher Employees

* Tuition Reimbursement Form for CUB Employees

* Tuition Reimbursement Form for Public School Administrators and Supervisors Association (PSASA)

* Tuition Reimbursement Form for Unaffliated Employees

Highly Objective State Standards of Evaluation (HOUSSE) forms-

* For middle, secondary, and pre-K-12 (Art & Music) teachers (207 KB) The purpose of this form is to obtain Highly Qualified (HQ) status using HOUSSE. HOUSSE is only available to professionally certificated teachers hired prior to the first day of the 2006-2007 school year. Information on NCLB and requirements for new teachers is available in the certification and NCLB areas on the MSDE website, www.marylandpublicschools.org This forms should be submitted to the Office of Certification.

* For early childhood or elementary teachers working in elementary schools (203 KB) The purpose of this form is to obtain Highly Qualified (HQ) status using HOUSSE. HOUSSE is only available to professionally certificated teachers hired prior to the first day of the 2006-2007 school year. Information on NCLB and requirements for new teachers is available in the certification and NCLB areas on the MSDE website, www.marylandpublicschools.org This forms should be submitted to the Office of Certification.

* For middle and secondary special education teachers (237 KB) The purpose of this form is to obtain Highly Qualified (HQ) status using HOUSSE. HOUSSE is only available to professionally certificated teachers hired prior to the first day of the 2006-2007 school year. Information on NCLB and requirements for new teachers is available in the certification and NCLB areas on the MSDE website, www.marylandpublicschools.org This forms should be submitted to the Office of Certification.

* For early childhood, elementary, and Alt-MSA assignments (257 KB) Obtain Highly Qualified (HQ) status using HOUSSE. HOUSSE is only available to professionally certificated teachers hired prior to the first day of the 2002-2003 school year. Information on NCLB and requirements for new teachers is available in the certification and NCLB areas on the MSDE website, www.marylandpublicschools.org
